Answer: i had it many years ago. about 6 months of accutane cleared it up completely although it left severe scarring i have not had a recurrence. good luck

I found out that I have Acne Conglobata which is the severe form of Acne. My face isn't that bad, its quite mild-moderate at the moment. I have started on Roaccutane. My doctor said it works 100%. Is it true?
Answer: No. It is not 100% effective. Some forms of Acne Conglobata are resistant to treatment, although they can be controlled with agressive treatment over time.
I hope he warned you about the drying side effects and possible depression associated with Accutane, and he will be monitoring your liver enzymes while you are on it.
Question: Acne conglobata and cystic acne...? I have always had bad acne since I was 10 yrs. old, and now I am 16. I've used all the OTC stuff, and doxycycline, minocycline, differin, tretionin(Retion-A) and others. They will help for a while, but not that great, and it will always comes back. Lately I have been getting big cysts and nodules, under my skin, right now i have about 25 and one is forming(OUCH!!) Some have been there for a few months. I am going to the dermatologist, again soon, in about a week. But I also had two cysts, on my chin that were connected under my skin. I would touch one, and pus and stuff would come out of the other. I was really freaked out, and never heard of it. I googled "interconnected cysts" and he only thing I found was, a severe for of acne, called acne conglobata. I have really bad acne, but only one occurrence, of the connected cysts, so do I have a case of acne conglobata, and what do I do, and also what should I tell my derm.? Any one have any similar problem?
Answer: Accutane works fantastic when everything else fails. I found that the SkinID from Neutrogena worked way way better than Proactive though.

Answer: I'm not a dermatologist, I'm just a medical student and i will try my best to answer ur question.
To treat acne, we must first understand the pathogenesis of acne. Acne is formed when there is excessive sebum production, clotted pores and the pores get infected by bacteria like Propionibacterium.acnes and Pityrosporon.ovale. Clotted pores(usually by dirt) and excessive sebum production(due to hormon imbalance esp during puberty) lead to accumulation of the sebum in the pores. Bacteria who infect this pores will convert the sebum to pro inflammatory fatty acid and this leads to inflammation in the lower follicular infundibulum. This inflammation is the produces the acne!
Therefore, to treat acne, 3 factors must be eliminated,namely sebum production,bacteria infection and clotted pores.
To reduce excessive sebum production, the best drug to do the job is known as Isotretinoin a.k.a Accutane. It is a vitamin A derivative. Although the exact mechanism of action is unknown, the drug is the most effective treatment avaiable today and it can only be prescribed by a certified dermatologist. It have several side effects such as dry lips,dry eyes and hyperlipidemia but all this effect can be reduced if the drug is used under careful supervision by the doctor. Another unproven side effect is suicidal tendencies but this side effect is very very rare. However, pregnancy should be avoided while taking Accutane as it can cause severe birth defects.
To eliminate bacteria infection, antibiotics is use orally and topically. Oral antibiotics such as tetracycline or Doxycycline is the typical antibiotic used for acne. It have synergistic effect if used together with Accutane, i.e. both drugs strengthen each other's effect. Topical antibiotic such as Metronidazole gel(Metrogyl or Flagyl) can be applied on the cystic acne. This will significantly reduce acne.
To unclot pores, the doctor will prescribe suitable facial cleanser depending on skin type. Adapalene a.k.a Differin is also prescribe to do the job. Skin may appear too dry initially but this is temporary.
With the 3 factors removed, acne will be significantly reduced by the 1st month of treatment.
